About the Role

This remote position is for a Pharmacy Care Coordinator who will act as a key point of contact between pharmacy operations and physician offices. The role centers on handling prescription clarifications, tracking pending requests, documenting every interaction accurately, and following up to keep cases moving quickly. Qualifications

  • Current pharmacy technician registration with the State Board of Pharmacy
  • A bachelor’s degree in business administration, accountancy, sales, marketing, computer sciences, or a related discipline, or equivalent practical experience
  • At least 3 years of experience in healthcare call centers, pharmacy settings, or social service environments
  • Clear verbal and written communication abilities
  • Working knowledge of MS Office tools, including Outlook, Word, Excel, and Access
  • Strong organization and time-management skills
  • Self-driven approach with a high level of accuracy and detail orientation
  • Ability to work with little supervision
  • Preferred understanding of disease states and pharmacy care workflows

Responsibilities

  • Communicate professionally with physician offices by phone and written messages, keeping information accurate and easy to understand
  • Record all conversations, clarifications, and follow-ups in the Pathpoint Rx system
  • Handle internal questions quickly and work toward resolution within required timelines
  • Place outbound calls to physician offices to request refills or obtain updates on open clarifications
  • Review prescription clarifications and refill requests to confirm they are complete and correct
  • Follow all applicable policies, procedures, safety standards, and regulatory requirements
  • Support patients and pharmacy care coordinators with medication therapy and pharmacy-related concerns, while taking physical, emotional, and social factors into account
  • Take on additional duties as needed to help the team meet objectives and improve workflow
