About the role
We are currently seeking a Nurse Practitioner to join our team.
Reporting to the Director, Ambulatory Services and Maternal Child the successful candidate will:
Engage in direct and in-direct advanced clinical practice, education, research and consultation within the geriatric diversion clinic to ensure the highest quality of care is provided to patients and families. Actively participates as part of the program and professional leadership team in promoting quality care by implementing evidence based practices, evaluating patient outcomes, development and evaluation of policies, clinical programs and services. Model both the professional and organization core competencies and provide expert clinical nursing leadership.
What you’ll be doing:
- Uses advanced theoretical knowledge base and clinical competence and skill to assess, plan, provide, and facilitate care for a geriatric population of patients across a continuum of care.
- Assesses patients/families through multiple strategies, including comprehensive admission history and interview, advanced physical examination, BPMH and medication reconciliation, interval assessments and the interpretation of diagnostic testing to determine need for medical, nursing, or other care.
- Utilizes the nursing process to determine patient response to their health state.
- Provides leadership in professional and patient population learning and education by: Developing teaching programs and conducting educational teaching sessions with patients/groups and or families, in hospital or in community/public forums with focus on health promotion, illness prevention, and the impact of the illness experience on patient and family.
- Contributes to the body of nursing/specialty knowledge by: Providing leadership in the critical analysis and review of literature and the application of research findings to practice through the development and implementation of clinical practice guidelines and standards of care.
- Utilizes advanced preparation and knowledge of patient population needs, processes of patient care, care systems, the research process and professional practice to provide and support leadership in the following areas: The development, implementation, and evaluation of quality management programs/initiatives, care delivery models, practice standard/guidelines/policies and procedures and associated health outcomes and resource utilization, in collaboration with the professional practice and nursing leadership teams.
What you bring:
- Registered and in good standing with the College of Nurses of Ontario (CNO) required
- Masters in Nurse Practitioner, or Masters in Nursing/Master of Science in Nursing with successful completion of a post Masters Acute Care Nurse Practitioner program from a recognized school of graduate studies required
- Membership with Nurse Practitioner’s Association of Ontario
- Minimum one (1) – three (3) years experience as Nurse Practitioner in geriatric and/or emergency specialty required
- Minimum three (3) years relevant clinical specialty-related experience as a Registered Nurse required
- Experience in Geriatric and Chronic Diseases Management required
- BCLS required
- RNAO Membership preferred
- Excellent physical assessment skills
- Demonstrates excellent problem solving, decision making and critical thinking skills
- Demonstrates excellent organizational and time-management skills
- Demonstrates initiative and the ability to function both independently and as an interprofessional team member in a fast paced demanding environment
- Excellent interpersonal, communication and public relations skills
- Demonstrates excellent clinical skills and professional leadership capabilities with a focus on collaborative teamwork, patient flow and quality improvements
- Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all
- Good working knowledge of Electronic Documentation
- Commitment to providing an exceptional experience for patients, family and staff
- Demonstrated commitment to, and understanding of the principles of diversity, equity and inclusion
