RN Case Manager

Location: Washington, DC

Schedule: Monday–Friday, 8:00 AM–5:00 PM 40 hours/week

Pay Rate: $36.32–$50.00/hour

Position Overview

Under the direction of the Supervisory Care Manager, the RN Case Manager is responsible for providing comprehensive care coordination services to medically fragile children and young adults enrolled in the health plan. The RN Case Manager supports members with complex medical, behavioral health, and psychosocial needs by conducting assessments, developing care plans, coordinating services, and addressing barriers to care.

Caseload Structure

• Standard RN Case Manager Caseload: Approximately 75 members Mix of Level 1, Level 2, and Level 3 acuity members
• Specialty RN Case Manager Caseload: Approximately 60 members Primarily high-complexity Level 3 members

Acuity Levels

Level 1: Stable, well-managed members requiring minimal intervention

Level 2: Moderate healthcare utilization and support needs

Level 3: High-risk, high-utilization members requiring intensive care management

Field & Office Expectations

• Approximately 3 field visits per week
• Remaining time spent completing documentation and care coordination
• Flexible scheduling for field visits
• Typically 4 days per week in the field and/or office
• Opportunity to attend member well-child visits and provider appointments

Required Qualifications

• Active DC Registered Nurse (RN) License
• Associate Degree in Nursing or higher
• Minimum 2 years of Case Management experience preferred
