RN Home Care Coordinator

Location: Washington, DC (Onsite)

Pay Rate: $95,000 – $100,000 annually

Shift: Mon – Fri / 8:00 AM to 4:30 PM

Job Type: Full-Time / Direct Hire – Permanent Placement

Job Summary

P3SG is seeking an experienced and detail-oriented Home Care Coordinator (RN) to oversee and manage in-home care services for an elderly population.

This role is responsible for ensuring high-quality, compliant, and well-coordinated care delivery by supervising home care staff, managing service schedules, and collaborating closely with an interdisciplinary team (IDT).

The Home Care Coordinator plays a key role in care planning, staff oversight, service coordination, and quality improvement initiatives.

Key Responsibilities

• Participate in interdisciplinary team (IDT) meetings, assessments, and care planning
• Coordinate and oversee all in-home care services for assigned participants
• Supervise personal care attendants, home care staff, and RN support
• Ensure accurate home care scheduling and adequate staffing coverage
• Monitor performance and service quality of external home care agencies
• Maintain accurate, up-to-date clinical documentation and service records
• Manage and resolve participant complaints per policy guidelines
• Support Quality Assurance Performance Improvement (QAPI) initiatives
• Provide on-call support during non-operational hours
• Assist with additional clinical coordination duties as needed

Required Qualifications

• Registered Nurse (RN) – Required
• Minimum 2+ years of home care experience
• Minimum 1+ year working with elderly or frail populations
• Experience in:
• Care coordination or case management
• Nursing assessments and clinical care
• Strong communication, organizational, and leadership skills
• Ability to work effectively in a fast-paced, team-based environment

Preferred Qualifications

• Experience in home health or geriatric care settings
• Prior supervisory or team lead experience
• Familiarity with interdisciplinary care models and quality improvement processes
