Job Description
Job Title: Registered Nurse (RN) – Ambulatory Care Clinic (ACC), Long-Term Care
Position Summary:
The Registered Nurse (RN) – Ambulatory Care Clinic (ACC) provides professional nursing services to residents within a long-term care environment who require clinic-based assessment, treatment, and follow-up. The RN works collaboratively with physicians, nurse practitioners, and the interdisciplinary team to support chronic disease management, acute care needs, and continuity of care, while promoting resident safety, dignity, and quality of life.
Key Responsibilities:
- Perform comprehensive nursing assessments, including physical, cognitive, and functional status
- Triage residents based on urgency and clinical presentation
- Deliver nursing care and treatments for acute and chronic conditions (e.g., wound care, infections, pain management)
- Administer medications, injections, and therapeutic interventions in accordance with physician or NP orders
- Assist with and support medical procedures and examinations
- Monitor, evaluate, and document resident responses to interventions and adjust care plans as needed
- Collaborate with interdisciplinary team members to coordinate and optimize resident care
- Communicate effectively with residents and families regarding care plans, changes in condition, and follow-up
- Facilitate continuity of care, including transitions to and from acute care or specialist services
- Maintain accurate, timely, and compliant documentation in health records
- Adhere to infection prevention and control standards and organizational policies
- Participate in quality improvement, risk management, and resident safety initiatives
Qualifications & Credentials:
- Current registration and licensure as a Registered Nurse in good standing with the applicable provincial regulatory body (e.g., College of Registered Nurses)
- Bachelor of Science in Nursing (BSN) preferred
- Current Basic Life Support (BLS) certification required
- Advanced Cardiac Life Support (ACLS) certification considered an asset
- Certification in Gerontology Nursing (e.g., CNA or equivalent) considered an asset
- Wound Care Certification or equivalent training preferred
- Minimum of 2–3 years recent nursing experience, preferably in long-term care, geriatrics, or ambulatory care
Knowledge, Skills & Abilities:
- Strong clinical assessment and critical thinking skills, particularly in geriatric populations
- Knowledge of chronic disease management and age-related health conditions
- Ability to manage complex and changing health conditions in a clinic-based setting
- Excellent communication and interpersonal skills with residents, families, and team members
- Strong organizational and time management skills in a fast-paced environment
- Proficiency with electronic medical records (EMR) systems
- Demonstrated commitment to person-centered care, safety, and ethical practice
Working Conditions:
- Ambulatory clinic setting within a long-term care facility
- Frequent interaction with elderly residents, including those with cognitive impairments
- Regular standing, walking, and occasional physical assistance of residents
- Day or rotating shifts based on operational requirements

Pay: $48.00-$60.00 per hour
