Registered Nurse Case Manager (Hybrid)
Location: Washington, DC (Georgetown area)
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (4 days onsite, 1 day remote)
Pay Rate: $47-$49/hour
Position Overview
Seeking an Registered Nurse Case Manager to support a complex patient population with chronic conditions such as diabetes, hypertension, and COPD. This role focuses on care coordination, discharge follow-up, and ongoing patient engagement while ensuring compliance with NCQA standards.
Key Responsibilities
- Manage a caseload of up to 45 patients with high-acuity needs
- Conduct telephonic clinical assessments and bi-weekly patient outreach
- Coordinate care across providers, specialists, and community resources
- Support hospital discharges and ensure safe transitions of care
- Develop and manage individualized care plans in line with NCQA guidelines
- Identify care gaps and connect patients to necessary medical and social services
- Collaborate with interdisciplinary teams and participate in case reviews
Required Qualifications
- Active DC Registered Nurse License (required at time of hire)
- Minimum 2+ years of nursing experience
- 1-2 years of case management experience
- Experience working with high-acuity or frequently hospitalized patients
- Familiarity with NCQA standards and guidelines
