Duties
- Conduct comprehensive wound assessments, including measuring, documentation, and evaluation of healing progress using advanced EMR (Electronic Medical Record) systems such as Epic and Cerner.
- Develop and implement individualized wound care plans utilizing evidence-based practices, including dressing changes, debridement, and application of specialized therapies like botulinum toxin treatment when appropriate.
- Administer medications, including topical agents and injections, following strict aseptic techniques to promote healing and prevent infection.
- Perform advanced procedures such as suturing, catheterization, IV infusion, and ostomy care with precision and adherence to infection control standards.
- Educate patients and their families on wound management techniques, nutrition for healing, pain management strategies, and lifestyle modifications to support recovery.
- Collaborate with multidisciplinary teams across acute care settings like ICU, emergency medicine, long-term care facilities, hospice & palliative medicine programs, and outpatient clinics to coordinate seamless patient care.
- Maintain meticulous medical documentation in compliance with HIPAA regulations while tracking vital signs, laboratory results, ICD coding (International Classification of Diseases), and other diagnostic evaluations.
Requirements
- Valid LPN or RN license in good standing; additional certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or specialized wound care certifications are highly preferred.
- Proven experience in wound care management within hospital settings, long-term care facilities, or outpatient clinics.
- Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
- Strong knowledge of anatomy, physiology, infection control protocols, and aseptic techniques essential for safe patient care.
- Experience with complex procedures such as suturing, catheterization (including urinary and vascular), tube feeding management, and specimen collection/processing.
- Ability to perform physical examinations accurately while assessing patient needs in diverse environments such as medical-surgical units or specialty clinics like geriatrics or pediatrics.
