Job Description

The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

This position will primarily serve on our Gastrointestinal Oncology clinic.

What You’ll Do

  • Design and manage therapeutic plans using the most effective, economical, and evidence-based treatments for oncology patients in the ambulatory setting.
  • Collaborate across disciplines with oncologists, infusion teams, and specialty pharmacy to ensure seamless care transitions and optimal medication outcomes.
  • Provide direct patient care through assessments, collaborative drug therapy management, and medication monitoring.
  • Assist in ordering and interpreting medication-related tests to guide therapy decisions and ensure safety.
  • Educate patients and caregivers on treatment plans, side effect management, and wellness strategies.
  • Document care plans in the medical record to support continuity and quality of care.

Minimum Qualifications

  • Education: Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.
  • Experience: PGY-1 residency required or equivalent clinical experience.
  • Licensure/Certifications: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged.

Preferred Qualifications

  • Education: Pharm.D. from an ACPE accredited College of Pharmacy. Degree Required.
  • Experience: PGY-2 residency in Oncology or equivalent clinical experience.

Physical Demands – Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease.

Onboarding Requirements

Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen.

The starting base rate for this role is $64.37 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
