Substance Abuse Counselor
Summary Description of Position:
The primary role of the Counselor will be to facilitate, and appropriately document, individual, group, and family therapy sessions; work with individual clients in developing goals and writing individualized treatment plans; work with clinical and medical team for continuity of care and best client outcomes from admission to aftercare.
Responsibilities
Ø Interview clients, referral resources, family members and significant others to gather needed information essential in ensuring proper treatment and discharge planning
Ø Facilitate daily substance use education, relapse prevention, and other psychoeducational groups
Ø Provide individual counseling services to clients receiving treatment for substance abuse and co-occurring mental health disorders
Ø Record daily progress notes for each encounter with clients
Ø Utilizes assessment and interview skills to determine appropriate treatment plan and protocol
Ø Complete clinical/biopsychosocial assessments
Ø Develop and implement individualized treatment plans that reflect the identified needs of the client
Ø Work continuously with Clinical and Medical teams to ensure continuity of care
Ø Provide crisis counseling as needed
Ø Formulate and coordinate Discharge Planning to include clinical discharge summary
Ø Conduct family therapy as warranted to support the identified needs of the client
Ø Maintain contact with family representative of client and ensure updates are provided with a valid Release of Information
Education/Experience:
Ø Bachelor’s (Required)
Licensure:
Ø CSAC, LPC, LCSW, LSATP, LCP or equivalent (Required)
Knowledge of:
Ø Theory and practice of individual, group and family counseling as pertaining to those with mental health and substance use disorders.
Ø Various Substance Use inventories/assessments
Ø Motivational Interviewing techniques, relapse prevention
Pay: From $25.00 per hour
