This position is responsible for:
Manages and monitors assigned caseload of Turning Point Center for Women and Children participants to coordinate care and services both within Turning Point Center as well as with other external social service providers. Responsible for case management of resident families to ensure that both the parent and child(ren)'s physical, mental social, and educational needs are addressed within a strength based, trauma informed and empowerment framework.
Develops individual service plans that include residents plans for vocational, educational, housing, financial, family and mental and physical health, where applicable. Additional support and planning may be needed to address any substance abuse and/or mental health challenges that may arise during their participation in the program. As applicable, help to determine eligibility for and apply to other mainstream resources within the DC Metro Area may be needed. Evaluating & documenting participants' qualifications, progress, stagnation and other related activities related to the participants program participation and the Pathway of Hope program will be needed. The Case Manager I will serve as the primary advocate for the participate to function effectively in an environment within Turning Point Center and beyond. Ensures that Salvation Army policies and procedures, and any contractual requirements, are met by providing excellent services and doing the most good.
Key Responsibilities:
- Responsible for coordinating Participants services (i.e., medical, mental health, housing, educational, and vocational supports).
- Execute 1:1 case planning, preparing individual service plans, participating in the interdisciplinary team meetings, aftercare, and discharge plans, completing progress notes documentation, and meeting with each program participant twice a week, conducting one in-home inspection and one case management meeting to ensure comprehensive support and progress tracking.
- Support with new participant information and orientation sessions; to incorporate move-in and move-out home inspections along with providing assistance for basic cleaning and life skills during weekly home inspections.
Physical Requirements and Working Conditions:
- Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) 5-10% of work time. Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work may be performed in a normal office environment within a climate-controlled structure. There is little, or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, and dirt.
- Bachelor's or Master's Degree from an accredited college or university in the behavioral or social sciences, or a related discipline; AND
- Two (2) years or more, within the past six (6) years of appropriate experience that demonstrates acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field for vulnerable populations; OR
- Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
Licenses and Certifications:
Valid Driver's License and must meet TSA MVR Requirements.
LGSW, LGPC in the DC Metro Area, preferred
