Shift Details
- Schedule: Monday to Friday, 5 x 8-hour shifts, 08:30 AM - 05:00 PM
- No on-call requirement; optional weekend shifts available
- Holiday shifts may be required: Memorial Day, Independence Day, Labor Day
Responsibilities
- Float throughout patient tower across MedSurg units as needed
- Care coordination, discharge planning, and needs assessment
- Order Durable Medical Equipment (DME)
Requirements
- Valid MD License
- Basic Life Support (BLS) Certification
- Minimum of 3 years of experience in case management
- Experience in an outpatient clinic and acute hospital setting
- Regulatory knowledge: CMS and HIPAA compliance
- Must have medical inpatient experience
- Flu vaccination required
