Social Worker (Community Outreach)

Our client is seeking a compassionate and dedicated Social Worker to join their Community and Social Care team. This position focuses on community outreach and providing essential support services to vulnerable individuals and families. You will be instrumental in assessing needs, connecting clients with appropriate resources, advocating for their well-being, and facilitating access to social services. The ideal candidate possesses strong interpersonal skills, a deep understanding of social welfare systems, and a commitment to empowering individuals and communities.

Key Responsibilities:

Conduct comprehensive assessments of client needs, including social, emotional, and environmental factors. Develop and implement individualized care plans in collaboration with clients and their families. Provide counseling and support services to individuals facing various challenges, such as poverty, domestic violence, mental health issues, or substance abuse. Facilitate referrals to community resources, government agencies, and healthcare providers. Advocate for clients' rights and needs within various systems, including legal, educational, and healthcare. Organize and lead community outreach programs and workshops to raise awareness and provide support. Maintain accurate and confidential client records, documenting interventions and progress. Collaborate with other professionals, including healthcare providers, educators, and law enforcement, to ensure holistic client care. Participate in team meetings, case conferences, and professional development activities. Respond to crisis situations with empathy and provide immediate support and intervention. Monitor client progress and adjust care plans as necessary. Promote social inclusion and support clients in achieving their personal goals and enhancing their quality of life.

Qualifications

Bachelor's degree in Social Work (BSW), Psychology, Sociology, or a related field. A Master's degree (MSW) is preferred. Minimum of 3 years of experience in social work, community outreach, or case management. Knowledge of social welfare policies, community resources, and relevant legislation. Excellent interpersonal, communication, and active listening skills. Strong assessment, problem-solving, and crisis intervention abilities. Ability to work independently and as part of a collaborative team. Proficiency in record-keeping and case management software. Empathy, cultural sensitivity, and a non-judgmental approach to client interaction. Willingness to travel within the local community to meet clients and conduct outreach. Valid driver's license and access to reliable transportation. This is an on-site role based in the scenic coastal city of Kochi, Kerala, IN .
