Senior Clinical Social Worker - Geriatric Care

This vital role focuses on providing advanced psychosocial support and case management to elderly patients and their families within a clinical setting. The Senior Clinical Social Worker will assess the complex needs of geriatric patients, develop individualized care plans, and offer counseling and therapeutic interventions to address emotional, social, and practical challenges. You will work collaboratively with physicians, nurses, and other healthcare professionals to ensure holistic patient care, facilitating access to community resources, advocating for patient rights, and providing crisis intervention when necessary. This position requires a strong understanding of the unique issues faced by older adults, including chronic illness, cognitive decline, and end-of-life care, as well as expertise in therapeutic modalities relevant to this population. Responsibilities include: Conducting comprehensive psychosocial assessments of geriatric patients and their families. Developing and implementing individualized treatment plans in collaboration with the interdisciplinary healthcare team. Providing individual, family, and group counseling and psychotherapy using evidence-based approaches. Assisting patients and families in navigating healthcare systems, accessing community resources, and managing practical concerns such as finances and housing. Providing crisis intervention and support in acute situations, including elder abuse and neglect cases. Advocating for patient needs and rights within the healthcare system and community. Facilitating discharge planning and coordinating post-hospital care services. Documenting all patient interactions, assessments, and interventions accurately and in a timely manner. Participating in team meetings, case conferences, and continuing education activities. Supervising and mentoring junior social work staff and interns. Qualifications: Master's degree in Social Work (MSW) from an accredited institution. Current and valid professional social work license (e.g., LCSW, LSW) as per state/national regulations. Minimum of 5 years of post-master's experience in clinical social work, with a specialization in geriatric care or medical social work. In-depth knowledge of aging processes, common geriatric conditions (e.g., dementia, Parkinson's), and associated psychosocial challenges. Proficiency in various counseling techniques, including cognitive-behavioral therapy (CBT), supportive counseling, and grief counseling. Strong understanding of healthcare systems, social welfare policies, and community resources relevant to the elderly. Excellent assessment, intervention, and case management skills. Exceptional interpersonal, communication, and advocacy skills. Ability to work effectively in a fast-paced, multidisciplinary team environment. Experience in crisis intervention and de-escalation techniques.
