0.5 FTE Personal Care Worker - (External)

Job Purpose:

The Personal Care Worker is a frontline role responsible for delivering high-quality, person-centered support to tenants within a 24/7 supportive housing environment.

This role combines direct care, mental health and substance use support, and operational leadership to ensure safe, consistent, and trauma-informed service delivery aligned with a Housing First and harm reduction framework.

• Delivering 'on - demand' supports and programs following a 'housing first' philosophy as part
• of a collaborative team.
• Following prescribed policies and procedures associated with the housing program.
• Supporting participants through a person-centered approach

The Personal Care Worker (PCW) will work with an interdisciplinary team to support participants of Brunswick Street - a supported housing program for senior to age in place with wrap-around supports. The interdisciplinary team provides 24/7 support to tenants around life skills and health and wellness to ensure participants can effectively manage their tenancy and lives.

Education and Experience:

Minimum 2+ years

• experience in:
• Mental health and substance use (mandatory)
• Harm reduction (mandatory)
• Emergency shelter and/or transitional housing settings (mandatory)
• Demonstrated ability to apply a trauma-informed approach

Assets:

• Formal education in social, health, addictions, human services or community work
• Lived/living experience related to the population served
• Cultural understanding and relevant training
• Diploma is an asset but not a requirement
• Lived and/or living experiences of homelessness, mental health and substance misuse an asset

Demonstrated experience working with individuals with complex health and social needs

• including substance use, chronic illness, cognitive decline, and social isolation, homelessness and/or barriers to stable housing
