Schedule: Monday-Friday 8 AM-5 PM
No Call!
Service Area: Fairfax and Prince William County area and 50 miles radius
Requirements
Must have at least 1 year of Home Health experience within the last 3 years as an LPN.
HCHB charting experience is also required.
Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications, injections, IV infusions, and manage catheter care following strict protocols and HIPAA regulations.
- Execute specialized procedures such as tube feeding, wound suturing, sterile processing, and oxygen therapy with precision and aseptic technique.
- Monitor patients’ conditions closely, including those with complex needs like dementia, Alzheimer's disease, or acute pain management cases; triage urgent issues promptly.
- Document all patient interactions accurately using EMR (Electronic Medical Records) systems such as Epic or Cerner; ensure compliance with ICD-9/ICD-10 coding standards for billing and reporting.
- Collaborate with interdisciplinary teams in hospice care, geriatrics, pediatric or neonatal settings to develop personalized discharge plans and coordinate ongoing home & community care.
- Provide education on health coaching, medication administration, infection control practices, and safety protocols to patients and their families.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Proven experience in home health nursing or related settings such as hospitals (ICU experience preferred), nursing homes, assisted living facilities, or emergency rooms.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques.
- Hands-on experience with IV insertion, phlebotomy, catheterization, ventilator management, and basic life support (BLS).
- Familiarity with EMR/EHR systems like Epic or Athenahealth; ability to accurately document patient data and medical records.
- Ability to work independently in patients’ homes while maintaining high standards of safety and infection control.
- Excellent patient assessment skills combined with compassionate communication tailored to individuals with disabilities or cognitive impairments such as dementia or traumatic brain injury.
