Job Summary
Carepath Support Services, a new home care business in Niagara Falls, is seeking a dedicated RN or RPN for a part-time, on-call role. As we grow, this flexible position will be available for patient assessments, care coordination, and urgent needs. We are seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including hospitals, nursing homes, assisted living, and home care environments. This role requires a dedicated professional with strong clinical skills and the ability to work effectively with diverse patient populations, including pediatrics, seniors, and individuals with dementia or chronic conditions. The RPN will collaborate with multidisciplinary teams to ensure optimal patient outcomes and uphold the highest standards of nursing practice.
Duties
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor, assess, and document patient conditions using medical terminology and electronic health records systems such as Epic or Cerner
- Assist with pain management, case management, and patient education to promote recovery and well-being
- Perform phlebotomy and laboratory procedures as needed for diagnostic testing
- Provide comprehensive patient care in various settings including hospital, acute care, long-term care facilities, assisted living, and home care environments
- Support patients with special needs such as dementia care, senior care, pediatric care, or working with children
- Collaborate with physicians, therapists, and other healthcare professionals to develop individualized care plans
- Maintain accurate documentation of all nursing activities in compliance with healthcare regulations
- Ensure infection control protocols are followed diligently to promote a safe environment for patients and staff
- Assist in medical office tasks when required, including patient intake and vital signs measurement
Skills
- Proven experience in hospital settings, acute care, or nursing home environments; experience in pediatrics or senior care is highly desirable
- Proficiency in electronic health record systems such as Epic or Cerner
- Strong knowledge of medical terminology, medication administration, and laboratory procedures including phlebotomy
- Excellent patient care skills with a focus on compassionate communication and cultural sensitivity
- Ability to work independently as well as part of a multidisciplinary team
- Experience in pain management, case management, dementia care, or home care is preferred
- Familiarity with medical office procedures and working with children is a plus
- Strong organizational skills with attention to detail in documentation and medication management
- Ability to adapt quickly to changing clinical situations while maintaining professionalism
