Job Description

Position: Clinical Case Manager - Services to Indigenous Population

Location: Sarnia Office

1 Permanent Full Time

Reporting to the Manager of Integrated Services, the incumbent will be responsible for the following functions:

  • Providing client needs identification through ongoing assessment and client directed care planning.
  • Implementing client centered plan of care that focus on strengths, wellness and empowerment to support recovery and goal achievement.
  • Providing crisis prevention and intervention planning with the hope of preventing a relapse and/or hospitalization.
  • Acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed.
  • Liaising with family/significant other and natural supports to facilitate the clients’ links within the community

Qualifications

  • Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO.; Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a post-secondary degree with certification with a relevant professional registration (acceptable to the employer).
  • Experience engaging and working with Indigenous Peoples in a community setting.
  • Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population.
  • A demonstrated knowledge, awareness and sensitivity to Indigenous Cultures and Traditions.
  • Valid Ontario Driver’s Licence and access to a vehicle are required.

Hours of Work: 35 hours/week. Flexible hours are required to meet the position requirements.
