Inpatient Clinical Pharmacy Specialist Role

Join the Thunder Bay Regional Health Sciences Centre as a Clinical Pharmacy Specialist in the Inpatient Pharmacy Team. Focus on medication management, patient education, and quality improvement initiatives. The Clinical Pharmacy Specialist (CPS) will engage in key clinical pharmacy performance indicators, including medication reconciliation and developing pharmaceutical care plans. You will also resolve drug therapy issues, participate in patient care rounds, and enhance team collaboration. Mentoring staff and making critical decisions on medication management are essential aspects of this role. Key Responsibilities:

• Conduct medication reconciliation for patient safety
• Develop and implement pharmaceutical care plans
• Resolve drug therapy problems effectively
• Collaborate in interprofessional patient care rounds
• Educate patients about their medications Requirements:
• Doctor of Pharmacy degree
• Registered Pharmacist Part A with Ontario College
• Accredited Canadian Pharmacy Residency preferred
• Two years hospital pharmacist experience is an asset
