About House Calls
House Calls is an interdisciplinary primary healthcare team providing ongoing comprehensive geriatric home-based primary care since 2008. As a unique collaborative led by Clinical Director Dr. Mark Nowaczynski, VHA Home Healthcare, and Lead Administrative Agency SPRINT Senior Care, House Calls provides comprehensive primary medical care, as well as occupational therapy, physiotherapy and social work. House Calls also facilitates connections to community support services for homebound seniors with physical, cognitive and social frailties. Position Overview
The Intake Coordinator will be the initial point of contact for House Calls; this position will take responsibility for screening calls and determining needed services/supports to which clients (and their families) can be referred. The House Calls Intake Coordinator will be responsible for administrative and service responsibilities such as scheduling for the allied health and medical team members, managing fax referrals and filing. The House Calls Intake Coordinator utilizes their knowledge of medical terminology and medical ethics, and exercises judgment and decision-making to provide support for the House Calls team of primary care professionals.
Service Responsibilities
- Screen and assess clients’ eligibility for House Calls, and process new client referrals.
- Responsible for acquiring a general knowledge of all SPRINT Senior Care programs and specific knowledge of the agency’s boundaries and prime functions to respond to callers and/or visitors.
- Triage incoming calls to determine urgency of medical team's response, and liaise with the appropriate team member.
- Coordinate and chair weekly rounds, including preparing a list of new client referrals and existing clients for review, preparing the weekly agenda, and taking minutes.
- Appropriately and accurately document client information in the Telus MedAccess EMR, OceanMD, and the AlayaCare client information management system, advising the House Calls teams as appropriate.
- Responsible for faxing referrals and following up with specialist offices as needed.
- Maintain statistical records which track the referral sources and services/support to which clients are referred.
- Support research activities by compiling information to support House Calls research projects.
- Maintain accurate and complete documentation related to the Intake function.
- Maintain the House Calls vaccine fridges, including ensuring a successful Public Health annual inspection of the fridges.
- Coordinate equipment donations and maintain inventory.
- Order and maintain PPE and supplies and tracking inventory.
- Assist management in coordination of statistical reports, program planning and meetings.
- Generate reports and recommendations to improve service.
- Ensure that clients are receiving responsive customer service that is personalized, individualized and of the highest quality according to SPRINT Senior Care’s Continuous Quality Improvement (CQI) concepts.
- Schedule client visits for House Calls services.
- Initiate and/or reply to client/hospital/Ontario Health @Home and or other intake referral phone calls regarding service provision.
- Maintain the confidentiality of all client information to ensure client privacy is protected at all times.
- Prepare House Calls welcome packages, and respond promptly to requests for House Calls information to be mailed out.
- Provide clerical and support services for House Calls team members, including data entry, filing, photocopying, FAX transmissions, managing incoming and outgoing mail, and maintaining office supplies.
- Responsible for all incoming and outgoing mail and maintaining all House Calls office supplies.
- Other tasks as assigned.
Qualifications
- Certificate or diploma in Medical Terminology, or Medical Office Administration.
- Minimum 2 years’ experience working in Medical Office Administration.
- Experience working with and documenting in an Electronic Medical Record (EMR) is required.
- Previous experience in handling confidential or sensitive information; knowledge of applicable data privacy laws.
- Demonstrated excellence in written and verbal communications.
- Excellent organizational skills.
- Proficient in Microsoft Office suite.
- Experience using Telus MedAccess EMR, OceanMD and Alayacare is an asset.
- Ability to work cooperatively in a high-pressure team setting is essential.
- Flexible, creative approach, showing initiative.
- Good problem-solving skills and good judgment are essential.
- Ability to work without supervision and respond professionally to a high volume of incoming calls.
