General Description
The Case Manager is responsible for developing, implementing, and monitoring individualized person-centered plans for people receiving services. This role ensures that all aspects of care and support align with the individual's preferences, needs, and goals to promote autonomy, dignity, and empowerment. In addition to planning and clinical coordination, the Case Manager serves as the primary point of contact for managing personal benefits and entitlements to ensure the stability and security of all individuals. The Case Manager works collaboratively with individuals, families, and a multidisciplinary team to facilitate effective and tailored service delivery.
Job Duties
- Collaborate with individuals, families, and stakeholders to create comprehensive and individualized person-centered plans.
- Ensure plans reflect the individual’s preferences, goals, and needs while adhering to relevant regulations and standards.
- Update and revise service plans as necessary to respond to changing needs or life circumstances.
- Monitor and evaluate the effectiveness of the person-centered plans, making adjustments as needed.
- Conduct thorough assessments to gather information on an individual’s needs, strengths, and challenges.
- Manage and maintain all individual benefit accounts and public assistance programs, including obtaining original identification, verifying active payment status, and updating information with government agencies.
- Maintain accurate and up-to-date documentation of individual interactions, benefit statuses, plan updates, and outcomes.
- Prepare and submit reports as required by internal protocols or external regulatory agencies.
- Work closely with a multidisciplinary team including social workers, medical professionals, and community resources to facilitate effective service delivery.
- Facilitate regular meetings and maintain open communication between all parties involved in the individual’s care.
- Advocate for the rights and preferences of individuals within the service delivery system.
- Provide support and guidance to individuals and their families, addressing concerns and resolving issues that arise.
- Participate in ongoing training and professional development to stay current with best practices and regulatory changes.
- Perform other duties as assigned to support the program and its participants.

Knowledge & Abilities
- Strong interpersonal and communication skills.
- Ability to work effectively with diverse populations.
- Proficiency in using case management software and Microsoft Office Suite.
- Excellent organizational and problem-solving abilities.
- Knowledge of Maryland’s service delivery systems and regulations.
Education, Licenses, & Training
- Bachelor’s degree in social work, human services, education, healthcare administration, or a related field. Equivalent work experience may be considered.
- At least 1 year of experience in a similar role or related field.
- Experience with individualized planning or case management is preferred.
Shift
- Monday – Friday 8:30am – 5:00pm and flexibility to accommodate individual needs and schedule variations.
