About the Role

We are looking for a dedicated and patient-focused Clinical Pharmacist to join our healthcare team. The role involves reviewing medication therapies, promoting safe and rational use of medicines, collaborating with healthcare professionals, and supporting patient care initiatives to improve clinical outcomes.

Key Responsibilities

• Review medication orders for accuracy, appropriateness, and safety.
• Monitor patient medication therapy and identify potential drug-related problems.
• Participate in multidisciplinary patient care activities and clinical discussions.
• Provide medication counseling and education to patients and caregivers.
• Conduct medication reconciliation during admission, transfer, and discharge.
• Monitor and report adverse drug reactions (ADRs) and medication errors.
• Ensure compliance with hospital medication management policies and protocols.
• Support antimicrobial stewardship and rational drug use programs.
• Maintain accurate clinical documentation and medication records.
• Assist healthcare professionals with drug information and therapeutic recommendations.
• Participate in quality improvement and patient safety initiatives.
• Ensure proper storage, handling, and utilization of medications as per regulatory standards.

Requirements

• Pharm.D / M.Pharm (Pharmacy Practice) / B.Pharm with relevant clinical experience.
• 0–3 years of experience in Clinical Pharmacy or Hospital Pharmacy.
• Valid State Pharmacy Council Registration.
• Knowledge of pharmacology, therapeutics, and medication safety practices.
• Good communication and patient counseling skills.
• Ability to work collaboratively with doctors, nurses, and other healthcare professionals.
• Strong analytical and problem-solving skills.
• Basic knowledge of NABH standards and hospital quality practices is preferred.
