Primary Care Nurse Practitioner

An established, single-physician family practice on the Hamilton Mountain is looking to welcome a Nurse Practitioner to the clinic for four days per week. This role offers the chance to practice with a high degree of independence while still being connected to a broader, well-supported healthcare network.

As part of the Hamilton Family Health Team, the clinic has access to a range of allied health professionals, including mental health counselling, dietary services, and pharmacy support—helping ensure well-rounded patient care.

Practice Environment

You’ll be working alongside a small, experienced administrative team, allowing you to focus your time on clinical responsibilities. The clinic uses TELUS PS Suite for electronic medical records and emphasizes a collaborative, patient-focused approach.

What You’ll Do

In this role, you will provide comprehensive care to patients of all ages. Responsibilities include:

• Performing detailed health assessments and developing care plans
• Managing both short-term concerns and ongoing medical conditions
• Prescribing medications and ordering/interpreting diagnostic tests within your scope
• Delivering preventive care and health education
• Supporting patients with chronic disease management, including diabetes
• Providing care across areas such as pediatrics, prenatal care, women’s health, mental health, and geriatrics
• Administering immunizations and addressing acute health issues
• Collaborating with interdisciplinary team members when appropriate
• Maintaining accurate documentation within the EMR system

What We’re Looking For

• Current registration as a Nurse Practitioner in good standing with the College of Nurses of Ontario
• Minimum of 3 years’ experience in a primary care setting
• Strong clinical judgment and communication abilities
• Comfortable working independently and managing a patient panel
• Experience with EMR systems (TELUS PS Suite is an asset)
