About the role:

The Social Worker educates members on improving health outcomes, assesses Social Determinants of Health (SDOH) and community resource needs, participates in process improvement and other pilot programs as they arise, and works with other Oscar teams to ensure exceptional care for our members.

  • Assess and support SDOH needs of our members, and educate members on resources for community support programs.
  • Participate in crisis intervention for members.
  • Serve as a member advocate and assist with problem solving, as well as addressing any barriers to care or compliance with the care plan.
  • Communicate with members in person, via phone and/or secure messaging to provide education on health conditions, new medications, and procedures.
