Role Description

This is a full-time, on-site role for a Nurse Practitioner, based in Oakville, ON. The Nurse Practitioner will provide patient-centered primary care, performing health assessments, diagnosing and treating illnesses, managing chronic conditions, and developing care plans. They will collaborate with interdisciplinary healthcare team members while maintaining accurate records and ensuring compliance with healthcare standards. Additionally, the Nurse Practitioner will support patient education, preventative care, and health promotion initiatives.

Qualifications

• Proficiency in conducting health assessments, diagnosing conditions, and managing acute and chronic illnesses
• Experience in patient education, health promotion, and preventative care
• Strong documentation, record-keeping, and compliance with healthcare standards
• Collaboration and teamwork abilities within interdisciplinary healthcare teams
• Effective communication and interpersonal skills to support patient relationships
• Current registration and certification as a Nurse Practitioner in Ontario
• Knowledge of and experience with community health programs is beneficial
• Proficiency in electronic medical record (EMR) systems and basic computer skills
• Commitment to continuous learning to improve healthcare delivery and patient outcomes
