Care Manager / Clinical Lead – Special Projects (Registered Nurse)
Modern Angels Healthcare is seeking an experienced and highly skilled Registered Nurse (RN) to step into a Clinical Nurse Manager / Clinical Lead role supporting complex care delivery across hospital, community, and retirement care settings.
This role is ideal for a nurse with strong acute care hospital experience, combined with community or home care exposure, who is ready to take on a leadership role focused on care coordination, quality improvement, and clinical oversight.
Position Overview The Clinical Lead will oversee nursing operations and support the delivery of safe, high-quality, patient-centered care across multiple care environments.
This includes managing complex client cases, supporting care transitions from hospital to home or retirement settings, and ensuring continuity of care across interdisciplinary teams.
The role also involves supporting clinical quality initiatives, staff supervision, and compliance with healthcare standards.
Key Responsibilities
- Provide clinical leadership and supervision to nursing and support staff
- Oversee complex and high-acuity patient care across acute, community, and retirement settings
- Coordinate patient care plans across hospitals, physicians, families, and community partners
- Support safe and effective hospital discharge and care transition planning
- Ensure compliance with healthcare regulations, infection control, and safety standards
- Develop and implement clinical policies, procedures, and best practices
- Monitor quality indicators and support continuous quality improvement initiatives
- Participate in audits, reporting, and accreditation requirements
- Provide mentorship, coaching, and clinical support to staff
- Manage workflow coordination, staffing support, and resource allocation
- Collaborate with multidisciplinary healthcare teams to support patient outcomes
Qualifications
- Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO)
- Minimum 2 years of acute care hospital experience (required/preferred)
- Minimum 2 years of experience in community care, home care, or retirement care
- Strong experience in complex care, geriatrics, and care coordination
- Experience in hospice or pediatric care is an asset
- Proven leadership or charge nurse/supervisory experience preferred
- Strong understanding of Ontario healthcare system and care pathways
- Excellent clinical judgment and decision-making skills
- Strong communication skills with ability to work with families and multidisciplinary teams
- Ability to manage high-pressure clinical situations professionally
