Social Worker

Karuna Evam Shanti Vahini (KeSV) Foundation (www.karunashanti.in) works to improve health in communities, focussing on the most marginalized, who face barriers in accessing healthcare. Our approach is to identify and build leadership within the community so that they can support others facing chronic illness in their community. We also help patients connect with hospitals and other services and make sure they receive next levels of care and follow up. Our work has a special focus on family caregivers. We are starting a project in Uttarakhand providing home-based palliative care for people living with long-term serious illness.

About the position

There are openings for full-time positions based in Dehradun and Almora. Internet connectivity and comfort with virtual meetings (zoom, google meet etc) would be essential. The Social Worker will report to the Team Lead based in the district, and work would involve visiting patients in their homes. The candidate needs to be caring and have good people skills.

Qualifications

-A minimum Bachelor’s Degree in /Social Work/Development Studies

-Experience in working with community for at least 2–3 years. Experience in community health/palliative care projects will be an advantage

-Experience of working with a range of stakeholders and working with teams

-Fluency in Hindi is needed. Knowledge of local dialects would be an advantage

-Willingness to attend training outside the district or even the state is essential.

-Having own transport/ two–wheeler for local travel is desirable

Key Responsibilities

-Work along with the field-based Palliative Care team of KeSV Foundation to address medical and psycho-social needs of patients and their families

-Along with the Nurse, provide home-based palliative care to chronically ill patients and undertake follow up visits, and accompanied visits to healthcare facilities in case of referrals/ admissions

-Plan and co-ordinate Doctors’ home visits for patients enrolled in the project in consultation with the Team Lead and other team members

-Support in linking patients and families to eligible government schemes, and livelihood opportunities

-Maintain patient files and information, attend regular online and offline meetings with the field team and core team as and when scheduled

-Work closely with key partners including government, NGOs etc, and health functionaries at RH, CHC, PHC, and SC; ASHAs, ANMs, Multi-purpose workers, etc., and with the Foundation's core team located in Delhi/ Mumbai

-Train family caregivers and community members on how to care for chronically-ill patients

-Documentation of activities undertaken and regular reporting of work status and home visits

-Advocacy with local stakeholders as and when required
