Job Overview

We are seeking a dedicated and experienced Nurse Practitioner to join our family healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including pediatrics, senior care, and acute care environments. This role involves collaborating with the physician and other healthcare professionals to deliver high-quality medical services, manage patient cases, and support overall wellness initiatives. The Nurse Practitioner will utilize their expertise in medical terminology, medication prescribing, and patient care to improve health outcomes and enhance patient satisfaction.

***This position is for 2-3 four hour clinics per month throughout the year focusing on complimenting the doctor's patient roster. The month of August would entail potentially 2 four hour clinics per week. Vacation and conference coverage throughout the year will offer more hours as well.***

Responsibilities

• Conduct thorough patient assessments, including history-taking and physical examinations across diverse populations such as children, seniors, and patients in office.
• Diagnose illnesses, develop treatment plans, and prescribe medications in accordance with province regulations.
• Manage acute and chronic conditions, including pain management and dementia care.
• Utilize the electronic health records system, OSCAR, for documentation, order entry, and case management.
• Educate patients and families on health conditions, medication use, and preventive care strategies.
• Collaborate with multidisciplinary teams to coordinate patient care plans effectively.
• Ensure compliance with healthcare standards, safety protocols, and privacy regulations.

Experience

• Experience working with children (pediatrics) and seniors (senior care) and all ages in between.
• Skilled in pain management, case management, dementia care, and home health services.
• Proficiency in medical terminology, medication prescribing and mental health assessment, management and care.
• Familiarity with electronic health record systems , specifically OSCAR is advantageous.
• Prior medical office experience is a plus.
• Strong knowledge of nursing practices related to patient care across various specialties including pediatrics, geriatrics, and chronic disease management. Join our team to make a meaningful difference in patients’ lives through compassionate care and clinical excellence.
