About the Role

A growing telehealth organization is seeking a Clinical Pharmacist to support a diverse patient population across the Southeast region SPECIFICALLY FL, GA, NC, SC, TN, AL, MS, VA. MUST BE LICENSED IN ANY OF THE FOLLOWING STATES - FL, GA, NC, SC, TN, AL, MS, VA. This is a remote, patient-facing role focused on delivering high-quality, evidence-based care through medication management, patient education, and care coordination.

This position is ideal for a clinician who is passionate about improving patient outcomes, engaging patients meaningfully, and practicing at the top of their license in a collaborative, technology-enabled environment.

Key Responsibilities

  • Conduct comprehensive Medication Therapy Management (MTM) reviews
  • Perform medication reconciliation (Med Rec) and identify therapy gaps
  • Provide patient education on medications, disease states, and treatment plans
  • Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD)
  • Utilize motivational interviewing techniques to improve adherence and patient engagement
  • Collaborate with providers to optimize medication regimens and care plans
  • Identify and resolve medication-related problems and risks
  • Document all patient interactions accurately and in compliance with program requirements
  • Support care transitions and ensure continuity of care

Qualifications

  • Doctor of Pharmacy (PharmD) required
  • Active pharmacist license in at least one of the following states:FL, GA, NC, SC, TN, AL, MS, VA
  • Willingness to obtain additional state licenses as needed

Preferred Qualifications

  • BCPS, BCGP, or other BPS certification strongly preferred
  • Experience in:
  • Medication Therapy Management (MTM)
  • Ambulatory care
  • Chronic disease management
  • Patient education and engagement
  • Experience with care management programs (e.g., CCM, TCM, RPM) is a plus
