The primary function of the Community Health Advocate is to liaise and coordinate with key stakeholders and work with target populations specific to the Public Health Departments ongoing initiatives and programs aimed at improving the health of residents of the City of Youngstown. Key stakeholders include the Mahoning County Pathways HUB, Youngstown Health Improvement Zones, medical providers, City of Youngstown Departments, and other agencies identified by Minority Health Director and Health Commissioner. Examples of Duties Assists in connecting residents in target populations to resources and education offered by the public health department.
This work may include but is not limited to: Front-line healthcare worker advocating for the underserved populations Overseeing the work in the Youngstown Health Improvement Zones Connecting residents to needed and available resources through the health departments and other city departments Coordinating with key stakeholders in the community to provide residents with necessary care and resources Assists in the supporting and educating residents. This work may include but is not limited to: Work with target population to collect data on health needs and disparities Inform the community of health disparities Monitor and report the health status of the community they serve Provide culturally and linguistically appropriate education materials to residents Hold focus groups, conducts surveys and hold open forums and other outreach events to collect data on the health needs of the residents they are serving Coordinates with Community Health Advocate Supervisor. this work may include but is not limited to: Preparing and submitting monthly reports Reporting data back to the Community Health Advocate Supervisor to analyze, plan and implement strategies to address the needs Supporting other team members as needed Minimum Qualifications The following education , skills and experience are required for this role: High School Diploma or GED; A valid certificate as a Community Health Worker or will obtain the certificate within the first 6 months; Valid Ohio Drivers License and ability to utilize an operable, fully insured vehicle to work purposes.
Ability to pass a fingerprint, background and drug screening check. Have the ability work independently, maintain effective relationships with other employees and the public. Applicants must meet the above required qualifications to be considered EXAMINATION INFORMATION: TYPE:TRAINING AND EXPERIENCE EVALUATION: Applicant's grade will be determined based on Education and Experience found in the application and resume.

BONUS POINTS WILL BE ADDED TO PASSING SCORES (70%) ON THE FOLLOWING BASIS: 20% - MILITARY (DD-214 showing 180 days of active duty service and proof of honorable discharge required) 15% - Reserve Component - upon completion of entry-level training. 15% - PROOF OF CITY RESIDENCY (Submit four (4) different proofs of residency from one (1) year ago and four (4) different current proofs of residency - total of 8) Proof of Residency must be scanned and emailed to civilservice@youngstownohio.gov NOTE:ALL COPIES OF DIPLOMAS, LICENSES, CERTIFICATES, AND RESUMES MUST BE UPLOADED AT THE TIME OF FILING. Original college transcripts are required as verification of educational attainments
If you are a veteran and wish to claim the military preference credit, please upload a PDF COPY of FORM DD-214 showing proof of honorable discharge. *****As provided for in Youngstown City Ordinance ORD-11-244 and in accordance with the City of Youngstown Charter, Section 52 (E) any candidate who is a bona fide resident of the City of Youngstown for at least one year immediately prior to an entry level examination and remains a resident citizen of the City of Youngstown throughout the selection process, as determined by the Civil Service Commission, at the time of filing his or her application for examination, shall, if a passing grade is attained, have a credit of fifteen percent (15%) added to his or her raw score. The Applicant must show proof of residency by providing the following: Driver's License, Bank Statements, Utility Bills, Mortgage Release/Lease, and/or Insurance Statements.
In order to be considered applicants must submit four (4) different proofs of residency from one (1) year ago and four (4) current proofs of residency (Total of 8)
