Job Summary

The Home Care Nursing Coordinator is responsible for coordinating, managing, and supervising all home nursing care services. The coordinator will ensure timely staff deployment, patient satisfaction, quality care delivery, staff scheduling, documentation, and effective communication between patients, families, nursing staff, hospitals, and management.

Qualifications

• B.Sc Nursing / GNM Nursing.
• Valid Nursing Council Registration preferred.
• Minimum 3 years of clinical nursing experience.
• Minimum 1-2 years of supervisory or coordination experience in home care, hospital, or healthcare services.
• Computer proficiency in MS Office, Google Sheets, email, and healthcare software.

Staff Management

• Arrange and deploy qualified nurses and caregivers according to patient needs.
• Prepare and manage staff duty schedules and shift rosters.
• Handle emergency staff replacements during absences or emergencies.
• Monitor staff attendance, punctuality, and performance.
• Guide and support nursing staff in clinical and non-clinical matters.
• Conduct regular staff meetings and training sessions.

Patient Care Coordination

• Receive and process new nursing service enquiries.
• Assess patient requirements and identify suitable nursing staff.
• Coordinate patient admissions and service commencement.
• Ensure continuity of care and proper handover between shifts.
• Monitor patient satisfaction and address concerns promptly.
• Conduct regular follow-up calls and home visits when required.

Quality Assurance

• Ensure all nursing services comply with company standards and clinical protocols.
• Monitor quality of care through home visits and patient feedback.
• Investigate and resolve service complaints.
• Implement corrective actions and continuous improvement initiatives.
• Maintain infection control and patient safety standards.

Documentation & Reporting

• Maintain accurate patient records and service documentation.
• Ensure all assessments, care plans, and reports are completed on time.
• Prepare daily, weekly, and monthly operational reports.
• Update patient and staff information in company software and records.

Business Development Support

• Maintain good relationships with hospitals, doctors, and referral partners.
• Support marketing and business development activities when required.
• Promote additional home healthcare services to existing patients.

Emergency Response

• Be available for emergency service coordination.
• Handle urgent patient and staff issues.
• Coordinate emergency staff deployment and patient support.
