Your Opportunity:
We are seeking a Social Worker II to join Supportive Living, providing comprehensive behavioural and cognitive assessments for adults and seniors living in congregate settings. Supportive Living offers a non-institutional alternative for individuals who can no longer live independently, with a focus on health promotion, well-being, independence, and maintaining functional abilities. This role supports the Behaviour Support Team (BST), which assesses the underlying causes of responsive behaviours and psychosocial symptoms, recognizing behaviour as communication of unmet needs. Assessments consider each person's history, preferences, and environment, along with physical, psychosocial, and care-related factors. BST interventions are non-pharmacological and emphasize collaboration with Contracted Service Providers (CSPs), families, caregivers, and the interdisciplinary team to build capacity and improve client outcomes. The Social Worker II practices within a competency framework emphasizing professional accountability, systems thinking, creative problem-solving, communication, and relationship building. The role requires advanced skills in behavioural and cognitive assessment, development of individualized Behaviour Support Plans, and consultation with CSPs responsible for implementation. Responsibilities also include creating communication strategies, proactive support plans, positive dementia care approaches, safety planning, behaviour contracts, motivation programs, and providing mentorship and coaching to care providers, families, and caregivers. Significant experience addressing capacity concerns is essential, and designation as a capacity assessor is preferred.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
A Bachelor of Social Work is required. Basic Cardiac Life Support- Healthcare Provider (BCLS HCP). Communication skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment. Responsible to provide proof of active registration with the Alberta College of Social Workers (ACSW) and to maintain registration throughout the duration of employment. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
Preferred Qualifications
Education or training in mental health or geriatrics is an asset. Palliative Care Experience. Connect Care Experience. Recent experience working in the community with older adults who have complex needs.
