Senior Social Worker - Elderly Care

Our client is seeking a compassionate and experienced Senior Social Worker to join their dedicated team, focusing on elderly care services in **Pune, Maharashtra, IN**. This role requires a deep commitment to improving the lives of senior citizens by providing essential support, counseling, and advocacy. You will be responsible for assessing the needs of elderly individuals and their families, developing care plans, and connecting them with appropriate community resources and services. This position involves direct client interaction, case management, and collaboration with healthcare professionals and other social service agencies.

Responsibilities: Conduct comprehensive assessments of elderly clients' physical, emotional, social, and financial needs. Develop and implement individualized care plans in collaboration with clients, families, and healthcare providers. Provide counseling and emotional support to clients and their families navigating challenges related to aging, illness, or loss. Facilitate access to community resources, including healthcare services, housing assistance, financial aid, and recreational activities. Advocate for clients' rights and needs within healthcare systems and community settings. Maintain accurate and confidential client records, documenting assessments, interventions, and progress. Collaborate with a multidisciplinary team, including doctors, nurses, therapists, and other social workers. Organize and facilitate support groups for seniors and their caregivers. Stay informed about relevant legislation, policies, and best practices in elder care and social work. Respond to crisis situations and provide immediate support and intervention when necessary. Supervise and mentor junior social workers or interns as required.

The ideal candidate will possess excellent communication, interpersonal, and problem-solving skills, with a strong understanding of the unique challenges faced by the elderly population. A Bachelor's or Master's degree in Social Work or a related field, along with relevant professional experience, is required. This is an on-site position in Pune, requiring your physical presence.
