Mental Health and Dementia Independent Contractor

Role Description The Mental Health and Dementia Independent Contractor will provide client-centered support and services to individuals living with mental health challenges and/or dementia, as well as their caregivers. This is a part-time hybrid role based in the Greater Toronto Area, Canada, with a primary focus on in-person visits in the community from client home or care environment. Day-to-day tasks include direct client support, contributing to informal assessments, transportation and reporting and coordinating resources and referrals to appropriate services. The contractor will document client interactions, maintain professional boundaries, and collaborate with healthcare providers, community agencies, and families to ensure continuity of care. The role also involves advocating for clients’ needs, providing caregiver coaching, and participating in case discussions or team meetings as required.

Qualifications

• Demonstrated knowledge of mental health conditions and dementia, including symptom management, care planning, and risk assessment.
• Demonstrated knowledge of dementia, including progression and reaponsive behaviours
• Experience providing direct client support, counseling, or coaching to individuals and families in community or clinical settings.
• Strong communication, active listening, and interpersonal skills, with the ability to build trust with diverse clients and caregivers.
• Ability to work independently as an independent contractor, manage a flexible schedule, and prioritize multiple cases.
• Comfort with hybrid work, including home-based documentation, virtual sessions, and in-person visits across the Greater Toronto Area.
• Proficiency in maintaining accurate, confidential records and using basic digital tools
• Post-secondary education in social work, psychology, counseling, nursing, gerontology, or a related field.
• Experience working with older adults, caregivers, and marginalized communities is preferred; additional language skills are an asset.
• Access to reliable transportation for community visits within the Greater Toronto Area.
