Gattuso Centre for Social Medicine and Population Health

The Gattuso Centre for Social Medicine seeks to address health inequities, working alongside community organizations, UHN clinicians, marginalized populations, persons with lived experience, patients, and all levels of policymaking to co-design, evaluate, and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN’s most medically and socially complex patients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity.

Position Summary

The University Health Network is committed to high-quality and equitable care for all patients. UHN recognizes the significant health disparities experienced by patients with unmet health and social needs and the importance of providing care outside of traditional hospital settings — including in the community and at home. In response, UHN has established the Gattuso Centre for Social Medicine to transform how care is delivered to marginalized populations.

UHN is seeking Registered Nurses (RN) with specialized training in wound care to join our Social Medicine Program. This role will focus on addressing the social determinants of health for patients with complex medical and social needs, including those who frequently use hospital services and face barriers to care due to poverty, aging, being under-or-unhoused, mental health challenges, and substance use. Working as part of a broader interdisciplinary team, the RN will provide high-quality, compassionate care directly in community-based settings at 750 Dundas St. W., Dunn House (90 Dunn Ave), and the Social Medicine Mobile Health Clinic Vans.

The Registered Nurse (RN) with expertise in caring for patients with complex medical conditions, knowledge in chronic wound care, diabetic foot ulcer management and  focused clinical assessments, will collaborate with the Social Medicine Nurse Practitioner, Community Health Workers, Peer Support Workers, UHN care teams, and community organizations to deliver integrated, person-centred care that meets patients where they are. The successful candidate will play an active role in shaping the scope and direction of nursing practice within the Social Medicine Program, offering a rare opportunity to help define how care is delivered in this innovative space.

Duties

  • Community-Based Clinical Care: Deliver primary and episodic care to patients in non-traditional clinical environments, including the Stabilization and Connection Centre and Social Medicine Hub at 750 Dundas Ave., mobile health vans, and Dunn House. Provide wound care, chronic disease management, harm reduction support, health promotion, and linkage to additional care
  • Autonomous Nursing Practice: Use sound clinical judgment to assess, monitor, and respond to patient needs independently
  • Mobile Outreach & Navigation: Support outreach services by attending scheduled stops with the Mobile Health Clinic team and engaging clients who may not access traditional health care services. Provide navigation support, health and wellness education and assist with referrals to UHN and community services
  • Health Equity Advocacy: Identify systemic barriers to care and advocate, alongside community organizations, City and Province, for access to care and services including housing, mental health supports, and substance use programs. Build therapeutic relationships with individuals who may be mistrustful of traditional healthcare systems
  • Team Collaboration & Communication: Work closely with Social Medicine NPs, CHWs, Social Workers, and Navigators to deliver high quality care, support escalation and continuity of care. Participate in team huddles, interdisciplinary care planning, and case reviews
  • Flexible Service Delivery: Adapt to changing environments, community needs, and patient presentations. Be comfortable providing care in unpredictable and dynamic settings
  • Documentation & Reporting: Maintain timely, accurate clinical records in the electronic medical record system. Participate in data collection, evaluation, and quality improvement initiatives

 

  • Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
  • BScN preferred; additional certification or training in mental health, addictions, offloading, complex wound management, or chronic disease management is an asset
  • 3+ years of clinical experience, preferably in community health, primary care, harm reduction, emergency, mental health, crisis nursing and/ or street outreach nursing
  • Demonstrated expertise in chronic wound care (e.g. pressure ulcers, diabetic foot ulcers, venous/arterial ulcers) and lower extremity assessment and management
  • Strong clinical assessment skills and demonstrated ability to work autonomously and confidently exercise clinical judgment
  • High-level of comfort with structurally marginalized populations, including those who experience being unhoused, mental health challenges, substance use, and chronic illness
  • Familiarity with community-based health and social resources, social determinants of health, and commitment to trauma-informed, culturally safe, and anti-oppressive care
  • Valid Ontario Driver’s License (G-class) and ability to travel across Toronto for patient visits, as required. Mileage reimbursement will be provided in accordance to UHN policy
  • Ability to drive the Mobile Health Clinic van would be an asset (Requires 3-years' experience driving a cargo van)
  • Excellent interpersonal, communication, time management, and documentation skills
  • Ability to build relationships with patients and community partners
  • Ability to excel in non-traditional clinical settings, and is passionate about advancing health equity
