Wage Range: $48.51 - $60.64 per hour
Hours: 37.5 Hours per Week
Shifts: Variable: Days, Evening, Weekends
Status: Permanent Full-Time

Position Summary
This position will focus primarily on centralized inpatient pharmacy support. The successful candidate should enjoy working in a fast paced environment in close collaboration with other health care team members.
For this role, we are looking for a motivated, team-oriented individual with strong interpersonal skills. The successful candidate must possess sound knowledge in the provision of pharmaceutical care, have superior inter-professional, communication and organizational skills and demonstrated teamwork in a patient care setting.
Duties
- Review and dispense all doctors' medication orders for appropriateness
- Identify, resolve and prevent drug related problems
- Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines
- Conduct medication reconciliation
- Follow up on medication incidents and promote safe medication practices on the unit
- Respond to drug information questions; providing direct and/or functional supervision of technician staff
- Performing other duties consistent with the job classification, as required
- Possess a Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent; required
- Certificate of competency from the Pharmacy Examination Board of Canada; required
- Active registration with the Ontario College of Pharmacists (Part A); required
- Hospital pharmacy practice experience with patient-centered care experience in surgery; preferred
- Successful applicants must have an active registration with OCP (Part A)
- A Faculty of Pharmacy University appointment, a hospital pharmacy residency certificate, and an active membership in CSHP and other professional societies are all assets
- Hospital Pharmacy residency; an asset
- Applicants should have experience in an acute-care hospital pharmacy or ambulatory pharmacy setting
- Clinical experience with patient-centered care in oncology would be an asset
- Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
- Excellent verbal and written communication skills
- Excellent customer service and interpersonal skills
- Excellent organizational and time management skills
