We are currently seeking an experienced Nurse Practitioner (NP) to provide virtual clinical oversight and interim primary care support for a defined community care area. • Review changes in condition and recommend next steps
  • Support care planning and escalation decisions
  • Collaborate closely with nursing and therapy teams
  • Conduct in-person visits only when clinically necessary and pre-approved

Work Structure
