• Deliver patient-focused nursing care in home settings
• Ensure care adheres to best practice standards
• Assess and address the evolving needs of patients
• Collaborate with healthcare teams effectively
• Travel between patient residences as needed Requirements:
• Minimum of two years nursing experience required
• Good standing with the College of Nurses of Ontario
• Proficient in English, with additional languages as an asset
• Valid driver’s license and reliable vehicle
