Position Summary

Shibogama First Nations Council seeks skilled and adaptable Nurse Practitioners (NPs) to provide culturally responsive, community-centered healthcare services to remote and isolated First Nations communities. This role combines onsite and virtual care to deliver holistic primary care, support health promotion, and prevent illness.

Full-time positions may include a virtual care component, where the Nurse Practitioner leads primary care virtual clinic days and provides consultation to Community Health Nurses (CHNs) for CTAS 4 and 5. Remote work leverages secure telemedicine platforms, such as the Ontario Telemedicine Network (OTN), to deliver patient care and consultations from outside the community. NPs collaborate closely with in-community Community Health Nurses to ensure seamless patient care, including pre-visit preparations and post-visit interventions. Virtual care schedules and workflows are designed to maintain flexibility and efficiency, supporting both the NP and the community's needs.

Part-time positions offer flexible rotations, including options such as two weeks on and two weeks off or month-long rotations. Opportunities for overtime are available during both in- community assignments and remote work periods.

The NP will collaborate with local healthcare providers, community leaders, and interdisciplinary teams to improve health outcomes and address systemic disparities in alignment with Shibogama's mission.
