Youth-Focused Social Worker — RCM Health Consultancy Inc. (Toronto)

RCM Health Consultancy Inc., a Toronto-based healthcare navigation and case management firm with over 30 years of experience, is seeking a qualified, registered social worker (MSW, RSW) with a passion for youth mental health and community engagement. In this role, you will design and launch teen support services for a low-income population of youth and teens, including facilitating regular group meetings that provide a safe, supportive space for connection, skill-building, and peer support. You will conduct intake assessments, develop age-appropriate programming, and build trusted relationships with teens, families, schools, and community organizations to ensure youth are connected to the resources they need.

The ideal candidate brings experience working directly with adolescents—particularly those facing socioeconomic barriers—along with strong group facilitation skills, cultural sensitivity, and a trauma-informed approach to care. You are a self-starter comfortable building a new service line from the ground up, and you thrive in a collaborative, mission-driven environment alongside nurses, nurse practitioners, and case managers. Registration in good standing with the Ontario College of Social Workers and Social Service Workers is required.
