Temporary Social Worker – Access to Care

Become a vital part of the care team at Providence Care Hospital as a Temporary Social Worker focused on patient well-being. This role emphasizes psychosocial assessment and discharge planning.

You will provide crucial support by assessing client needs and creating effective treatment plans within the Access to Care & Transitions department. Key Responsibilities:

• Perform crisis intervention and psychosocial assessments
• Lead discharge planning initiatives in collaboration with clients
• Develop and evaluate person-centered treatment goals
• Facilitate community referrals and support
• Provide psycho-education to families regarding treatment processes

Requirements

• Master of Social Work (MSW) from an accredited institution
• Current registration with the Ontario College of Social Workers
• Two years of social work experience in a hospital setting preferred
• Familiarity with electronic patient records is an asset
• Ability to work collaboratively and effectively communicate
