Become a vital part of the care team at Providence Care Hospital as a Temporary Social Worker focused on patient well-being. This role emphasizes psychosocial assessment and discharge planning.
You will provide crucial support by assessing client needs and creating effective treatment plans within the Access to Care & Transitions department. Key Responsibilities:
- Perform crisis intervention and psychosocial assessments
- Lead discharge planning initiatives in collaboration with clients
- Develop and evaluate person-centered treatment goals
- Facilitate community referrals and support
- Provide psycho-education to families regarding treatment processes
Requirements
- Master of Social Work (MSW) from an accredited institution
- Current registration with the Ontario College of Social Workers
- Two years of social work experience in a hospital setting preferred
- Familiarity with electronic patient records is an asset
- Ability to work collaboratively and effectively communicate
