Overview
We are currently seeking a skilled and compassionate Telehealth Nurse Practitioner, licensed in multiple U.S. states, to join our growing virtual care team. This role plays a vital part in providing services such as medication therapy management, chronic care support, and care coordination in close collaboration with partnering health centers. We immediately licensed Nurse Practitioners in Texas, Illinois, Connecticut, Indiana, New Hampshire, California, and Georgia.
Responsibilities
- Conduct virtual assessments and consultations via video or phone, reviewing patients' medical histories, medication regimens, and current health status.
- Provide patient counseling and education on preventive care, chronic disease management, lifestyle modifications, and medication adherence.
- Coordinate care and make referrals by working with health center care teams, specialists, and community-based providers to ensure seamless coordination of care, including referrals and follow-ups.
- Maintain documentation by accurately recording patient encounters and care plans in the electronic medical record system in a timely and compliant manner.
