Job Title: Outreach Case Manager

Reports to: Lead Clinical Case Manager
Salary: $58,000–$62,000 annually, inclusive of a comprehensive benefits package
The Outreach Case Manager plays a critical role on the two-person GTBID (Golden Triangle Business Improvement District) Homeless Outreach Team at Pathways to Housing DC. The team maintains a close collaborative relationship with the Pathways DC DHS outreach team, attending biweekly meetings and coordinating regularly to ensure continuity of care and seamless service delivery across systems. The Outreach Case Manager works alongside the Lead Clinical Case Manager to carry a defined caseload of GTBID-area clients and high utilizers of services, building relationships with individuals experiencing homelessness to support their transition into and maintenance of stable housing. This position implements evidence-based clinical practices to develop strong rapport with clients, motivating them toward readiness for services and successful goal completion.
Approximately 75% of the time will be spent in the community, directly providing services to persons living on the street; the remaining time is allocated to fulfilling administrative duties, such as documentation, case conferencing, care coordination and case management.
The team maintains an 8am–6pm service window using a flexed scheduling model: one staff on an early shift (e.g., 8am–4pm) and the other on a later shift (e.g., 10am–6pm). This position follows the GTBID holiday schedule. Team members are also required to participate in quarterly daytime and evening counts of individuals sleeping on the street in the Golden Triangle.

Location of Services: This position is primarily based at the GTBID main offices at 1120 Connecticut Avenue NW, Suite 260, Washington, DC 20036, with a secondary office at the Pathways Outreach Office at 945 G Street NW, Washington, DC. Staff are embedded within the broader GTBID team and are expected to integrate fully with GTBID leadership, ambassadors, and operational staff as part of daily work.
RESPONSIBILITIES
- Conduct thorough assessments to identify individual needs, including housing, healthcare, mental health, substance abuse, and other support services.
- Refer individuals to relevant resources and services such as emergency shelters, transitional housing programs, healthcare providers, mental health counseling, substance abuse treatment, job training programs, and legal assistance.
- Provide crisis intervention support, responding promptly to mental health crises, substance abuse relapses, or threats to housing stability, and offering immediate assistance and support.
- Assist individuals in navigating the housing process, including helping them complete housing applications, identifying affordable housing options, and addressing barriers to housing access.
- Develop and implement individualized case plans tailored to each client’s unique needs and circumstances, outlining specific steps and timelines for achieving goals.
- Respond to GTBID dispatch calls and address concerns raised by police, fellow social service providers, business owners, employees, residents, and downtown visitors regarding individuals experiencing homelessness, using established emergency dispatch protocols and procedures.
- Participate in crisis intervention and screening for psychiatric disorders; respond promptly to crises and to calls from GTBID stakeholders involving concerns about clients in need of assistance, and complete appropriate documentation and aftercare.
- Attend biweekly DC DHS outreach team meetings and maintain ongoing coordination with DHS outreach staff to support integrated service delivery, shared client care, and continuity across systems.
- Represent Pathways and the Golden Triangle Business Improvement District at trainings, community meetings, and other venues as needed.
- Complete tasks and responsibilities as assigned.
QUALIFICATIONS
- Bachelor’s degree in psychology, social work, or behavioral health from an accredited university required. Master’s degree in a related field is preferred. LGPC, LGSW, CAC and/or pertinent certification (RBT/harm reduction/certified mental health technician) preferred.
- Minimum 3–5 years of experience in homeless outreach, case management and/or counseling of people with psychiatric disabilities, histories of substance abuse and/or trauma, and individuals experiencing homelessness.
- Demonstrated knowledge of housing services, the DC coordinated entry system, mental health, and substance use disorders.
- Excellent organizational, oral and written communication, time management, and computer skills.
- Successful track record of working independently, demonstrating initiative and complex problem-solving skills.
