Northern Health are seeking applications from motivated and suitably experienced Nurse Practitioners to join the Heart Failure team in the Emergency Services Division.

Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.

Services are provided through our six main campuses:

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.

About the Role

The Heart Failure Nurse Practitioner is an advanced practice nursing role within the Heart Failure Team. This role is designed to support, complement and evolve with the service demands of the department.

The Nurse Practitioner provides autonomous and collaborative nursing care across clinical contexts and is responsible for advanced clinical decision-making, prescribing, referral, ordering diagnostics, and admission/discharge of patients, in line with their defined scope of practice.

As a leader in heart failure care, the Nurse Practitioner will act as a positive role model, provide clinical leadership, and contribute to the education and development of staff, patients, carers, and the community.

To be successful in this role you will have:

  • Master of Nursing enabling registration as a Nurse Practitioner.
  • Current registration as a Nurse and Endorsement as a Nurse Practitioner in Australia.
  • Current prescriber number and eligibility for a provider number.
  • Demonstrated experience as an endorsed Nurse Practitioner in a relevant field.
  • Advanced clinical knowledge in heart failure management.
  • Proven ability to lead, mentor, and educate within a multidisciplinary environment.
  • Strong interpersonal, communication, and problem-solving skills.
  • Commitment to evidence-based practice, safety, quality, and patient-centred care.
