Job Overview

VPACPlus, a partnership between Vital Path Addiction Clinic and Joe’s Specialty Pharmacy in Vancouver, is seeking a Licensed Practical Nurse (LPN) to join our growing team.

This unique role offers a dynamic split between clinic and pharmacy settings. Your mornings will involve travel throughout Vancouver, Burnaby, and Surrey in a company vehicle, delivering medications to partnered facilities while providing essential nursing care and support to patients.

You will play a key role in ensuring patient health, safety, and comfort while contributing to our mission of delivering exceptional, community-focused care. Our diverse patient population requires daily medication support, including insulin, OAT (Methadone/Kadian), injections, and transdermal patch applications (not currently but may be required at some point) .

Responsibilities

  • Pharmacy & Community Care:
  • Deliver medications to partnered facilities in a company vehicle
  • Administer medications and monitor patient responses
  • Provide compassionate, direct patient care in community settings
  • Collaborate with pharmacists, physicians, NPs and other healthcare professionals to optimize outcomes and ensure comprehensive care
  • Educate patients on medications and overall health management

Clinic Support:

  • Assist with clinic operations, including Medical Office Assistant (MOA) duties
  • Conduct detox assessments and check-ins
  • Other assessments, including CIWA, COWS, PHQ/GAD etc
  • Blood pressure checks
  • Injections, IM and SubQ
  • Minor wound care
  • Support doctors who may be working virtual and need someone onsite, as needed
  • Support patient flow and contribute to a positive clinic environment
  • Administer medications and monitor patient responses
  • Provide compassionate, direct patient care in community settings
  • Collaborate with pharmacists, physicians, NPs and other healthcare professionals to optimize outcomes and ensure comprehensive care
  • Educate patients on medications and overall health management

Qualifications & Requirements

  • Current Licensed Practical Nurse (LPN) license in Canada and in good standing
  • Valid Canadian driver’s license and clean driving record
  • First Aid and CPR certification
  • Strong skills in medication administration and patient assessment
  • Excellent communication skills for effective patient education and collaboration within multidisciplinary teams
  • Ability to work independently and within a collaborative team
