Job description
Jordan's Principle makes sure all First Nations children living in Canada can access the products, services and supports they need, when they need them. Jordan’s Principle can help with a wide range of health, social and educational needs, including the unique needs that First Nations Two-Spirit and LGBTQQIA children and youth and those with disabilities may have.
Position Summary: The Case Manager supports the Roseau River Anishinabe First Nation RRAFN child and family in culturally appropriate services enabling integrated and well-coordinated care, across the region’s primary, acute and community services. With a working knowledge of First Nations health approaches, protocols and culture, the Case Manager promotes interdisciplinary collaboration and communication and provides information and referrals to community and partner organizations. The Case Manager also maintains databases and reporting requirements. The Case Manager provides services for RRAFN children with complex needs and children who are aging out. The support will include in-person, phone-based, and electronic communication contact with persons requiring direction and guidance in areas such as assessment, service access, and the coordination of support services. This position will focus primarily on RRAFN youth, their families, and support systems.
Duties include:
· Provides support to children with complex needs and their families. Services provided include child development, respite care, crisis intervention, behavior management and life skills training, whether on a one-to-one basis or in a group environment. A key component of this position is to assist the client’s physical, social, emotional, and daily life skills development based on the seven domains of development.
· Plans, develops, implements, and evaluates recreational, social and or educational activities for children and their families.
· The case manager will strive to create a fun and enjoyable atmosphere to encourage client participation in programs.
· Ensure systems are in place to support the delivery of the model of service.
· Takes the lead in securing effective development, delivery, and review of services.
· Develops a strategy that ensures children are fulfilling their lifetime potential.
· Works in collaboration with the Regional Health Authority and Children’s Rehab Services to ensure client needs are met while work is underway to create efficiencies, shared training, assessment methodology and tools.
· Emphasizes health promotion and illness and injury prevention and provides appropriate anticipatory guidance.
· Uses clinical skills to assess the client’s health status to enable joint planning, implementation, and evaluation of appropriate interventions.
· Promotes and supports patient safety by addressing client safety issues and concerns as they arise.
· Coordinates appropriate care and equipment including case management with other community resources. Provides presentations and education programs to communities, families, groups, and individuals.
· Liaisons with family, medical and other resources, as required.
· Performs other related duties as assigned.
Education/experience required:
- Bachelor of Social Work (Registration with the Manitoba College of Social Workers is required).
OR
- Nursing Background (Registration with a Manitoban Nursing College is required).
Proof of licensing or registration with a regulatory body should be provided with application.
Some asset/ experiences include:
- Experience working with First Nations, preferably with children, youth, and families.
- Experience with case coordination and/or case management involving Indigenous communities in a culturally safe manner.
- Experience in promoting best practices in Indigenous health.
- Experience working in a fast-paced and evolving environment with a concentration in health.
- Experience in the use of software such as Microsoft Project, Word, PowerPoint, Excel.
- Experience working with First Nations individuals, communities or organizations on issues related to health and wellness.
- Experience coordinating client care for a wide range of health issues.
- Experience in the mapping of health and/or social systems and assets.
- Experience in the development of individual wellness plans, particularly for those living with complex health care conditions.
CONDITIONS OF EMPLOYMENT:
