Job Description
Empower hospice clients as a Social Worker in Fraser Health’s community services located in Port Moody, BC. This casual role provides social work assessment and clinical services to support clients and their families.
As an integral team member, you will conduct thorough psychosocial assessments and develop personalized treatment plans in collaboration with healthcare professionals. You will also ensure clients access necessary community resources while providing emotional support during their care journey.
