Job description

This Registered Nurse is located in the outpatient clinic of the Mental Health (MH) Integrated Clinical Community (ICC). The RN Care Manager is an integral member of the interdisciplinary team and works closely within the interdisciplinary team framework to help foster high quality care and recovery through appropriate management of the client's mental health by providing assessments, planning, interventions, evaluation of treatment and continuity of care.
