These Veterans often have complex, chronic illnesses, are homebound or isolated, or rely on caregivers. HBPC allows them to receive high-quality care in familiar, comfortable surroundings. The Nurse makes a meaningful difference helping Veterans achieve lifelong health and wellness where they live.
