The Opportunity
We are looking for a Seniors Intensive Case Manager to join our Community Development Team at CUPS.
The Seniors Intensive Case Manager, Legacy supports individuals obtain and maintain housing by providing affordable rental units in a centrally located residential building, which offers a compassionate and safe living environment. You will work with building residents in obtaining and maintaining housing stability, offering both individualized care as well as facilitating community-oriented support. You will be responsible for providing caring, dignified and trauma-informed case management for CUPS clients while supporting the achievement of housing stabilization via the strengthening and expanding of professional and natural support systems.
The Seniors Intensive Case Manager, Legacy will provide support to clients via CUPS Integrated Care service delivery model, connecting clients to internal CUPS service providers and external providers, creating a network of holistic support to achieve client created goals.
In this role, you will support up to 8 seniors, providing services to through in-home visits, community engagement, and Resource Room support. You will coordinate referrals, lead service planning, and collaborate on goal setting with these participants.
You will provide client-centered and strengths-based case management support to CUPS program participants as well as provide community-oriented programming to all residents of the Legacy building. The ideal candidate is passionate about housing as a human right and providing access and support for individuals experiencing housing insecurity and homelessness, including the unique needs of older adults and equity deserving groups. You will possess strong communication skills and attention to detail, and will contribute to CUPS culture of collaboration, dignity, accountability, and continuous learning. Candidates are expected to possess an understanding of trauma-informed care as well as knowledge of the Residential Tenancy Act, The Homeless Charter of Rights, as well as the Truth and Reconciliation Commissions’ 94 Calls to Action.

- Starting salary: $57,024.12 – 59,846.81 per year
Key Responsibilities
- Manage CUPS Legacy Resource Room, hosting programs of interest and benefit to tenants.
- Provide holistic support to participants including but not exclusive to case conferences, attending appointments, home visits, creation of care plans, supporting life enrichment and community connection activities.
- Meet with clients within designated time availability and creating a hub of resource materials & basic needs essentials available to participants.
- Promote community involvement within the building through new tenant welcoming/volunteer orientation and opportunities, program delivery, events, and soliciting feedback.
- Assist clients in defining initial strengths and needs and track progress with the client via follow up assessments to celebrate achievements and determine further goals.
- In collaboration with the client, develop a care plan (or update previous care plan) to meet the needs identified and connect the client to services internally and externally to achieve goals.
- Conduct required income testing with clients every year or as income level changes.
- Reach out to neighboring community resources (ex. Community Associations, Faith Groups and other local agencies) to connect CUPS clients with larger community events and opportunities.
- Accurately enter all required data points in organizational & external databases regarding the administration of care plans and crisis intervention funding to ensure accurate reporting.
- Coordinate the flow of client information to internal and external client care team members for seamless continuation of service in alignment with CUPS Integrated Care service delivery model.
- Practice crisis intervention and de-escalation skills to assist clients in self-regulation.
- Remain up to date on internal changes to programs, services and personnel to ensure accurate care plan development and warm hand-offs of clients to colleagues.
Employment Requirements
- Bachelor’s degree in social services (Social Work/Psychology) with 2-3 years’ experience or Diploma in Human Services with 3-5 years’ experience within the vulnerable sector.
What You Bring
- Previous experience working with vulnerable populations, including seniors and those with disabilities.
- Experience facilitating community-oriented programming. Prior experience with recreation, food skills, and financial empowerment an asset.
- In-depth knowledge of Calgary social and government services, knowledge of Calgary homeless sector services an asset
- Demonstration of effective client de-escalation and crisis intervention.
- Demonstrated skill working effectively within a multidisciplinary team.
- Knowledge and understanding of client service outcome tracking and previous experience with client assessment tools a benefit.
Work Conditions
- The work environment can be fast-paced and unpredictable, requiring flexibility and adaptability to meet client needs.
- This position involves periods of working alone.
- The role may involve exposure to challenging situations, including interacting with clients experiencing mental health or substance use issues.
- This role involves exposure to secondary traumatic stress including exposure to details of trauma history and events.
- This role will often work in community settings, sometimes outdoors or in spaces with limited amenities.
- Travel within the city or region is required to connect clients with housing, services, and resources.
- This position requires the ability to clean including sweeping, cleaning toilets and general tidying.
- This role requires strong personal safety awareness and adherence to organizational safety protocols.
- Access to transportation is preferred to facilitate mobility between CUPS & client locations as well as partner agencies.
This role will require candidates to provide an acceptable police background check including Vulnerable Sector screening.
