The Opportunity

We are looking for a Family Intensive Case Manager to join our Community Development Team at CUPS.

Family Intensive Case Managers at CUPS play a vital role in delivering compassionate, dignified, and trauma-informed care to clients and their families as they work toward long-term housing stability. Through a strengths-based approach, you will support individuals and families living in place-based housing by fostering housing and health stability, promoting skill development, and enhancing social and community connections.

Working within CUPS’ Integrated Care service delivery model, you will collaborate with both internal CUPS providers and external partners to build a holistic network of supports tailored to client-identified goals.

You will foster belonging and connection of the entire family unit within Community Development buildings by facilitating peer support, encouraging participation in social and skill-building activities, and strengthening natural and community supports.

CUPS Family Intensive Case Managers support up to 15 heads of household and their families, providing services to through in-home visits, community engagement, and Resource Room support. They coordinate referrals, lead service planning, and collaborate on goal setting with participants.

While based in an assigned building, you will be expected to provide support across other Community Development sites as needed. In addition, this role includes oversight and day-to-day coordination and cleanliness of place-based Resource Rooms to ensure they are accessible, welcoming, and responsive to participant needs

  • Location: Primarily scattered sites across Calgary, as well as onsite, CUPS - 1001 10 Ave SW
  • Reports to: Team Lead, Community Development
  • Contract type: Permanent
  • Schedule: Full-time, Mon to Fri, 40 hours per week
  • Starting salary: $57,024.12 – 59,846.81 per year

Key Responsibilities

  • Provide holistic support to participants including but not exclusive to case conferences, attending appointments, home visits, creation of care plans, supporting life enrichment and community connection activities.
  • Manage a Resource Room including hosting social and skill-building programming provided by both internal and external stakeholders with a family lens.
  • Provide culturally aware support to CUPS clients, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present.
  • Meet with clients within the Resource Room and create a hub of resource materials & basic needs essentials available to clients and their families.
  • Promote community involvement within the building through new tenant welcoming/volunteer orientation and opportunities, dynamic program delivery, client events and soliciting client feedback.
  • Assist clients in defining strengths and needs through CUPS standardized assessment, and track progress with the client via follow up assessments to celebrate achievements and determine further needs.
  • In collaboration with the client, develop a care plan (or update previous care plan) to meet the needs of each member of the family and connect the individual to services internally and externally to achieve goals.
  • Conduct required income testing with tenants every year or as income level changes.
  • Reach out to neighboring community hubs (ex. Community Associations, Faith Groups and other local agencies) to connect CUPS clients with larger community events and opportunities.
  • Remain up to date on internal changes to programs, services and personnel to ensure accurate care plan development and warm hand-offs of clients to colleagues.
  • Accurately enter all required data points in organizational & external databases regarding the administration of care plans, referrals and resources provided to ensure accurate reporting.
  • Develop and facilitate group-based sessions for parents, including but not limited to brain development, child social/emotional development, and practical parenting skills.
  • Collaborate with parents, external partners, community agencies, daycares and health providers in fulfilling family needs and advocating for children and families as needed to obtain required supports.

Employment Requirements

  • Diploma or Degree in Community Rehabilitation, Child and Youth Care, Social Work, Psychology or related human services equivalent.
  • 3-4 years’ experience working with children and families with complex needs.
  • Current Basic First Aid & CPR certification.

What You Bring

  • Exemplary understanding of child development & brain development for children 12 and under.
  • Practical knowledge of Brain Story, Children’s Ages and Stages, Childhood trauma and barriers related to parenting.
  • Strong communication skills (oral and written) in English.
  • In-depth knowledge of Calgary social and government services, knowledge of Calgary homeless sector services would be an asset.
  • Demonstrated skill working effectively within a multidisciplinary team.
  • Proficiency in client databases (ex. HMIS.) considered an asset.

Work Conditions

  • The work environment can be fast-paced and unpredictable, requiring flexibility and adaptability to meet client needs.
  • This position involves periods of working alone.
  • The role may involve exposure to challenging situations, including interacting with clients experiencing mental health or substance use issues.
  • This role involves exposure to secondary traumatic stress including exposure to details of trauma history and events.
  • This role will often work in community settings, sometimes outdoors or in spaces with limited amenities.
  • Travel within the city or region is required to connect clients with housing, services, and resources.
  • This position requires the ability to clean including sweeping, cleaning toilets and general tidying.
  • This role requires strong personal safety awareness and adherence to organizational safety protocols.
  • Access to transportation is preferred to facilitate mobility between CUPS & client locations as well as partner agencies.

This role will require candidates to provide an acceptable police background check including Vulnerable Sector screening, as well as an Alberta Child Intervention Record check.
